Pediatric Care Clause Examples for Any Agreement

Pediatric Care. Benefits are provided for routine physical examinations and diagnostic Services regardless of Medical Necessity and Appropriateness, and other items and Services.
Pediatric Care. Routine preventive (well-child) care is provided by CareHere for eligible children 2 years and older, including: (a) Performing scheduled periodic assessments of growth and development to determine if a child is growing and functioning in accordance with established milestones. (b) Performing nutritional assessments to determine if a child’s diet is sufficient for health maintenance and proper growth and development. (c) Delivering immunizations in accordance with guidelines from the CDC’s Advisory Committee on Immunization Practices (ACIP). (d) Providing counseling and instruction to parents.
Pediatric Care. Members may see a pediatrician for Well Child Care within the Plan’s network without a referral or prior authorization from either the Plan or the PCP.
Pediatric Care. Bringing Wellstar and AUHS’s exemplary pediatric care to more Georgians across the state through Children’s Hospital of Georgia and Xxxxxxxx’s dedicated pediatric care and emergency departments.

Accordingly, the frequency and eligibility of services is subject to change.

  • Health Care The Company will reimburse the Executive for the cost of maintaining continuing health coverage under COBRA for a period of no more than 12 months following the date of termination, less the amount the Executive is expected to pay as a regular employee premium for such coverage. Such reimbursements will cease if the Executive becomes eligible for similar coverage under another benefit plan.
