The employee' s anniversary date shall be the date on w hich the benefit level changes. (c) Wage employees shall be paid the rate of pay of their subst antive posit ion w xxxx on income protection. (d) Occurrence of Statutory or Declared Holidays during an employee' s absence on the Income Protection Plan shall not reduce the number of days of income prot ection eligibility. (e) Where possible, employees w ho are absent from w ork due to illness or injury must notify their immediate supervisor of such absence no later than the employee' s normal st arting t ime on the day the employee begins his/her absence. (f) Employees w ho are entitled to leave of absence on account of non-occupational illness or injury may obtain it on production of satisfactory application to the employee’ s manager. (g) Each employee w ho is absent for a period of more than four (4) consecut ive w orking days shall f ile w ith the application a sat isf actory certificate from a qualified, medical practitioner. Each employee shall be allow ed, if qualified, to apply for this leave of up to four (4) consecutive w orking days w ithout a doctor' s certificate provided that the total number of such uncertified days in any calendar year shall not exceed eight (8) days. (h) Employees, including employees absent as a result of an indust rial illness or injury, w ill be required to produce any medical certificate necessary w ithin the f irst f if teen (15) days of absence. Income Protection Plan. The insurance policy known as the Income Protection Plan presently in force for City employees shall be maintained. The Insurance premium shall be waived for unit members with 100 days* or more of accrued sick leave (regardless of the number of sick days used in the past year). * In converting days to hours refer to Article 10, Section F, Subsection 1(A), 2(A) and 2(B).
Income Protection Plan. (A) The Board agrees to provide benefits in the amount of 60% of the employee’s regular salary for a period of two (2) years in the event of sickness, and five (5) years in the event of accident, beginning on the 61stday of disability, not to exceed $5,000 maximum per month. (B) Any benefits received under Section (A) above shall be integrated with sick leave, workers’ compensation, social security benefits, and disability benefits paid from the Public School EmployeesRetirement System; which benefits shall offset the income provided under this section of the contract. Employees shall be required to use all available sick leave before becoming eligible for income protection benefits. (C) The Board shall provide the necessary sick leave to cover those days wherein an employee’s sick leave has expired and the beginning date that the income protection plan would become effective. In those situations wherein an employee becomes disabled [for a period exceeding ten (10) consecutive working days] and does not have sufficient sick leave days accumulated to cover the absence, the Board will advance sick leave days until the disability income insurance benefits become payable (maternity leave is excluded from this benefit). The advanced sick leave days shall be paid back to the District as the employee again earns sick leave days. If an employee leaves the employment of the District and has not repaid all the advanced sick leave days then the amounts still due will be deducted from the employee’s final pay. (D) The Board agrees to continue paying for health insurance, as outlined in Article 37; and dental insurance, as outlined in Article 38 herein, while an employee is receiving benefits under the income protection plan. (E) The District will, at various times and through various media, keep employees informed regarding income protection coverage, procedures and issues.
Income Protection Plan. 35.01 If during the term of this Agreement, the Company notifies the Union in writing technological change (defined as changes in equipment or methods of operation) has or will create a surplus of any job title in any work location which will necessitate layoffs or involuntary permanent reassignments of regular employees to different job titles involving a reduction in pay or to work locations requiring a change of residence, or if a force surplus necessitating any of the above actions exists for reasons other than technological change and the Company deems it appropriate, regular employees in the affected job titles and work locations may elect, in the order of seniority, and to the extent necessary to relieve the surplus, to leave the service of the Company and receive Income Protection payments described in this Article subject to the following conditions: (a) The company shall determine the job titles and work locations in which a surplus exists, the number of employees in such titles and locations who are considered to be surplus, and the period during which the employee may, if he or she so elects, leave the service of the Company pursuant to this Article. Neither such determinations by the Company nor any other part of this Article shall be subject to arbitration. (b) If the Company deems it appropriate, it may offer to regular full time employees, in job titles in which a surplus does not exist, the opportunity to leave the service of the Company pursuant to this Article. The job titles, job locations and the number of employees to receive the offer will be determined by the Company. Such offer to each employee shall be conditioned on the company’s obtaining a qualified replacement for that employee from the employees in surplus job titles. Employees who accept voluntary downgrades will have their pay reduced over a period of time, as provided for in Article 33, Reassignment Pay Protection Plan. The provisions of this paragraph (b) will not be implemented by the Company unless and until regular employees in the surplus job titles and work locations have had an opportunity to elect to leave the service of the company pursuant to paragraph (a) above. The transfer provisions of this paragraph are separate from and not governed by the transfer and vacancy provisions of this Agreement. Income Protection Plan. The Board shall provide an Income Protection Plan (Long Term Disability Plan) in which each Teacher will participate. The Board shall pay all premiums except for $1.00 per year, which will be paid by the teacher. The remaining cost shall be shared equally by the participants. The plan shall provide an amount equal to sixty-six and two-thirds percent (66 2/3%) of the Teacher's salary. The qualification period shall be ninety (90) calendar days. The LTD policy will reflect language allowing the employee to receive LTD benefits at the end of ninety (90) calendar days, regardless of the number of personal illness days the employee has accumulated. Teachers who are absent due to illness or injury for more than ninety (90) calendar days must apply for LTD coverage until they return to work. Every certified staff member must enroll in this LTD program.
Income Protection Plan. Provisions shall be made for allowing Association Members to join an income protection plan, at their own expense, with a company chosen by the Association. The Association will furnish the name of the company for this protection plan to the Business Administrator, as a matter of record. The Board assumes no responsibility either expressed or implied or otherwise for the performance, errors, omissions, or representations the company may make to a Member or the Association with reference to the plan.
Income Protection Plan. The School District will contribute up to the sum of $13.02 per month for each eligible teacher employed by the school district toward the purchase of an income protection plan. The cost of the premium not contributed by the School District shall be borne by the employee and paid by payroll deduction.
